Advance Health Care Directive

Also called a "living will" in some states, an Advance Health Care Directive (AHCD) is basically a power of attorney for health care decision-making: a principal names another person as their health care agent for purposes of medical decision-making in the event they are unable to understand, appreciate or communicate their medical choices. A "pull the plug" instruction is often made in an AHCD.
